- Love your doctor? Not happy with waiting room delays?
Please give us your opinion. If you are currently being treated please take a few
moments to fill out our Treatment Questionnaire. This document should be
printed as a blank form and filled out at home and sent or returned to the
office. It helps us serve our patients more efficiently. Treatment

- Need more info on your diagnosis? Use the Medical

page for shortcuts to web sites that have been reviewed by your doctor for
accurate patient information. We also have links to the area hospitals where you
can obtain info on your upcoming hospital visit.
- Use Email for answers !! Our Email is checked
daily to provide rapid feedback to your questions without having to wait on
the phone or until your doctor can return your call. You can communicate
directly to your doctor with questions that you forgot at your office visit
or to decide if you need to see another doctor for a new problem. Use your

- Current patients may check the status of
referrals for consultations, surgery, and tests. Use the links at the top of this page to go to the appropriate area for
access to the approved referral lists. You will use your initials to find
your approval. This protects patient confidentiality. These lists are updated
weekly. Please call us if your initials are not listed. Many health plans,
worker's compensation cases and all HMO's require PRE-APPROVAL before a test
(ie MRI scan) , surgery, physical therapy or second opinion consultation can
be scheduled by our medical assistants. We know this can produce delays of a
few days to more than 2 weeks. Please be patient if your doctor's medical
assistant has not yet scheduled the procedure the doctor has ordered while
awaiting authorization. Many PPO plans and all Medicare patients do not
require pre-approval and are usually scheduled within 1-2 days after your
office visit.
